Two things:
1. Quiz and survey are two different things for distinct purposes. Now, there are several Wordpress plugins that cater both. However, it is recommended that you chose separate plugins/solutions for each that are specifically meant for respective purposes.
2. There are many quiz and survey plugins for WordPress sites. And a lot of them are good. But which one is ideal for you would depend on how you’re planning to use them. Do you want to generate leads? Do you want to improve on-page engagement? Do you want to build a brand? Do you want to push for direct sales? Do you want to gather personal information about the visitors? What kind of visitors are you targeting? These are some questions that must be answered to know which quiz and survey plugins are good for you. After all, what’s good for one user might not necessarily be fitting for you.

That all said, of course, I wouldn’t want you to leave with such a vague answer as “it depends”.
Starting with surveys, there are a few solutions I would recommend…
Google Forms is at #1 on that list. Coming from Google, you know this one would be great. And it’s also free, unlike other tools that charge a premium.
Admittedly, Google Forms might not necessarily be very good looking, but they are highly functional. There are plenty of options and customization features. You can create unlimited surveys and get unlimited responses – FOR FREE. Plus, it is also super easy to easy.

Once you have the survey form ready, simply pick the final code and embed it where you want it on your site.
Google Form is definitely the best only survey tool. And that’s what you should go for. But if you’re looking for options, here are some other good names:
· Typeform
· Survey Monkey
· SurveyLegend
· Polldaddy
· Zoho Survey
Now coming to quiz maker for your WordPress site, my first recommendation would be Playbuzz.
It is one of the tools I stumbled across only recently. And it is amazing! There’s a reason why it’s used by VH1, HuffPost, TheChive, Sports Illustrated, and other big websites. Creating your quiz here is very easy. The primary purpose for publishers to use Playbuzz is to increase the interactivity of their site; more engagement means better conversion.

Next to Playbuzz, Thrive Quiz Builder seems like a fitting option. For WordPress users, Thrive is a well-known name. Its quiz builder is very robust, with enough customization options. Moreover, it offers good split testing features. So, if you’re looking to generate leads, this can work quite effectively.
Aside from these two, other good online quiz makers that I know of are Brandquiz, ViralPress, and OnionBuzz.
There are some good solutions (not necessarily just WordPress plugins) that I think are good for online quiz and survey.
But again, just because I find them good doesn’t mean they would be ideal even for you. So, take some time out and please do some basic research.
