Have you ever read something about yourself in your own medical notes that shocked you?

I saw a doctor I was referred to. He came in and smiled, sat down with his back to me and appeared to pore over my chart. Then he got up and left. I looked over at my chart and saw, “Refuses to answer questions regarding history.” When he came back in, I had typed into my phone, “I’m deaf. Your back was towards me so I didn’t think you were asking me anything.” I had assumed that my regular doctor passed on this information. I’ve since learned to make it clear.
